A Study of Platelet Antibody by Using Flow Cytometry

Background :
@EN Immune-mediated thrombocytopenia is a disorder characterized by immunologic-mediated destruction of platelets. Since alloimmunization can be difficult to define based on clinical criteria, it is important to have reliable laboratory test to
detect
platelet antibodies. In this article, we established reference ranges of mean channel values for platelet antibodies in normal controls and measured these antibodies in thrombocytopenic patients(platelet count <50, 000§§) by using flow cytometry,
in
order to analyze the etiologies of thrombocytopenia and to compare the positivity rate between platelet associated antibody and antiplatelet antibody.
@ES Methods :
@EN Blood samples were freshly drawn from 130 healthy controls and 160 thrombocytopenio patients and platelet suspensions were prepared by differentiation centrifugation. For platelet associated antibody, platelet suspensions were incubated with
FITC-F(ab')2 goat antihuman IgG. For antiplatelet antibody, patients sera were incubated with target platelets before addition of FITC-F(ab')2 goat antihuman IgG. Finally, mean channel values of platelet antibodies were measured by flow
cytometer(Becton
Dickinson, USA).
@ES Results :
@EN The reference ranges of mean channel values(mean+2SD) for platelet associated antibody and antiplatelet antibody were 366.0 and 401.0, respectively. Both platelet associated antibody and antiplatelet antibody were detected in 17 patients. In

patients, only platelet associated antibody was detected and in 5, only antiplatelet antibody was detected with low value of Kendall's consistence(0.45). The underlying causes of 48 patients with either platelet associated and/or antiplatelet
antibody
positivity were most likely transfusion(41). Drug(4), immune thrombocytopenic purpura(2), and systemic lupus erythematosus(1).

@EN Because of time factors, sensitivity and simplicity, it has been shown that the measurement of platelet associated antibody is more efficient than that of antiplatelet antibody, when diagnosing immune thrombocytopenia by flow cytometry(Korean
